抄録
目的
美容手術後の鼻翼変形は組織の過剰切除が原因の一つである。症状としては鼻孔縁のnotching、鼻翼の平坦化、鼻翼顔面溝が浅く不明瞭になるなどが挙げられる。私どもが行っている術後鼻翼変形に対する鼻翼修正術について報告する。
方法
2016年から2022年までに当院で鼻翼の術後変形のために修正をおこなった症例は15症例でした。男性1例、女性14例で、平均年齢は30.9歳でした。
鼻翼の形態を希望に近く整えるため、耳介軟骨移植術や局所皮弁を用いて修正を行った。方法は全身麻酔下に耳介から採取した軟骨片のカーブを利用し鼻翼に作成した皮下ポケットへ移植する。局所で皮膚が利用できる場合には利用する。
結果
鼻孔縁のnotchingの修正は鼻孔縁へ皮膚あるいは軟骨を加えることで改善できた。平坦化した鼻翼形態や不明瞭となっていた鼻翼顔面溝は、鼻翼へのカーブ状の軟骨移植術によって改善できた。1例で感染し移植物を除去した。
考察
鼻翼の術後変形に対し、局所皮弁、軟骨を中心とした組織移植術によって修正術を行った。組織不足が大きい場合には皮膚軟骨複合移植などの報告もある。当院では主に耳介軟骨移植により鼻翼形態の改善を図った。鼻翼の修正は非常に困難な領域で、まだまだ術式改善の余地が大きいと考えている。

Purpose
Nasal alar deformity after cosmetic surgery commonly involves over resection. Symptoms include notching of the nostril sill, flattening of the nasal ala, and a shallow and indistinct nasofacial groove. We report on our revision methods of nasal alar correction for postoperative nasal alar deformity.
Methods
From 2016 to 2022, there were 15 cases of revision due to postoperative deformity of the nasal ala at our clinic. There was one male and 14 female patients with an average age of 30.9 years.
In order to adjust the shape of ala as desired, auricular cartilage grafting and local skin flap were used to correct the deformities. Under general anesthesia, a piece of cartilage is harvested from the auricle, curved, and transplanted into a subcutaneous pocket created in the nasal ala. If local skin is available, it is used.
Results
The notching of the nostril margin could be corrected by adding skin or cartilage to the nostril margin. The flattened nasal ala morphology and obscured nasofacial groove could be corrected with a curved cartilage graft to the nasal ala; in one case, the graft was removed due to infection.
Discussion
Postoperative deformities of the nasal ala were corrected by local skin flap and cartilage grafting. There are also reports of skin-cartilage composite grafts for cases with large tissue deficiencies. In our clinic, we mainly used auricular cartilage grafts to improve the nasal ala morphology. We believe that correcting the nasal ala is a very difficult area and that there is still much to improve in the methods.
